Orchestrating seamless Care experiences for patients, Families & Providers


Patient-focused care delivery continues to shift from institutional to community settings, requiring the need for effective partnerships between providers and across sectors. Underpinning this shift is an emphasis on how Ontarians interact with the health system and how the patient experience can be a valuable gauge of system performance (Health Quality Ontario, 2015). 

While partnering with patients is a recognized strategy to improve safety and quality of care, more work needs to be done to:

  • Create a culture that supports partnership and collaboration
  • Provide supporting structures and policies, information, and tools
  • Ensure the purpose of the patient participation fits the needs, preferences, and capacity of those engaged (Patient Safety Institute, 2017)
